SharePROCEDURE FOR CONFIRMATION OF REGISTRATION WITH YOUR HOME ASSOCIATION

All players registered for/attending Summer Try Outs must be registered with their HOME ASSOCIATION.If you have registered before August 1, 2012, the Upper Canada Cyclones' registrar will be receiving
confirmation from your Home Association Registrar. If you registered on or after August1, 2012 you will be
required to have the Confirmation of Registration Form completed and signed by you Home Association's
president or designate. There will be no exceptions to this rule as we are not permitted to allow any
player to participate in Upper Canada Cyclones' activities unless they are registered with their Home
